Tested positive for COVID-19? ADPH would like you to participate in a survey

BIRMINGHAM, Ala. (WBRC) – The Alabama Department of Public Health (ADPH) is asking anyone in Alabama who has tested positive for COVID-19 to respond to an investigation.

The survey will take 5-10 minutes to complete all responses will be kept confidential. You can find a link to the survey by going here or by clicking here and clicking “Have you had COVID-19?” banner to access the survey.

ADPH says the investigation is aimed at learning more about the spread of COVID-19 and complementing the ADPH investigation and contact tracing efforts.
